Start a Notification of Death

Notify us in 3 simple steps.

  1. Enter the deceased's name plus at least 1 additional piece of information, such as their address or date of birth.
  2. Upload a digital copy of the death certificate. If you don’t have the death certificate now, you can provide it at a later time.
  3. Provide your name and contact details so that we can reach out to you with next steps.

Once we receive your submission, a T. Rowe Price Life Events Specialist will call you. Please note that T. Rowe Price can accept instructions affecting an account only from an authorized party, such as an executor or beneficiary. Even if you are not an authorized party, you may continue with this initial notification process.

For your convenience, we encourage you to use our online scheduler at the end of this form to set up a time and date for us to contact you.

By proceeding, you agree to the U.S. and Canada Privacy Notice and the Life Events Terms of Use.
